We would be very grateful if you could prepare the following documentation to accompany your patient:​
 

• Letter of referral

• Please include an OPG and/or relevant intra-oral radiographs.  If the OPG is on film, we will scan it and return it to the patient at the consult appointment.

• Please instruct your patients to take antibiotic cover if they need it for medical reasons, even for their initial consult appointment, as we invariably complete a full periodontal charting.
